Download the FY27 Operating Grant Application and Application for Financial Assistance. When you have completed and signed both documents, upload them to the online form below in the specified sections.
Gather all required attachments listed below and upload them to the online form below in the specified sections.
When ready, fill out Section A. General Information and attach required forms (Board List, Financial Statement, Written Summaries, IRS letter) in the specified sections.
Select submit when the form and all attachments are completed in their entirety.

Required Attachments
- FY27 Operating Grant Application
- Application for Financial Assistance
- Board of Directors/Advisory Board
- Most recent copy of a full year’s financial statements. Examples include:
- Audited financial statements, reviews or compilations
- Unaudited financial statements
- Federal form 990 (not postcard)
- Annual treasurer’s report approved by Board. Include your organization name on the top of the first page.
- IRS 501(c)(3) tax exempt status letter. Include your organization name on the top of the first page.

- One to three brochures
- Budget explanation for increases or decreases of 20 percent or more
For New Applicants (Or Any Organization Whose Name or Address Has Changed)
- Submit a form W-9 with current address and EIN
- Letters of support from two or more community leaders/representatives of organizations who are unaffiliated with the organization
